Thesis Abstract

The effective management of pediatric pain remains a significant challenge within clinical practice, particularly due to the multifaceted nature of pain experiences in children and the often fragmented approaches employed by healthcare providers. Recognizing the necessity for a comprehensive, patient-centered approach, this study aims to develop a robust conceptual framework that guides holistic pediatric pain management integrating biological, psychological, social, and environmental dimensions. The primary objective is to create a theoretical model that synthesizes existing evidence, clinical expertise, and patient perspectives, thereby informing practice, policy, and future research. Employing a descriptive, exploratory design grounded in qualitative and quantitative methods, the study recruited a diverse sample of 320 healthcare professionals—including pediatric nurses, physicians, child psychologists, and pain specialists—from tertiary hospitals across the region. Purposive sampling was utilized to ensure inclusion of practitioners with at least five years of pediatric care experience. Data collection involved semi-structured interviews with 40 key informants, structured questionnaires comprising 30-item Likert scales administered to all participants to assess attitudes, knowledge, and practices related to pediatric pain management, as well as systematic review and synthesis of existing literature. Validity and reliability of instruments were established via expert panel review and Cronbach’s alpha coefficient, respectively. Thematic analysis facilitated qualitative data examination, while structural equation modeling (SEM) was employed to test hypothesized relationships among identified components of the framework, using SPSS and SmartPLS software. Expected findings suggest significant interrelations among biological, psychological, and socio-environmental factors influencing pediatric pain perception and management outcomes. The study aims to identify key determinants—such as parental involvement, children’s developmental stages, and healthcare provider attitudes—that impact the implementation of holistic pain strategies. It is anticipated that SEM analysis will validate a multi-dimensional model, confirming the significance of integrating various domains into a unified conceptual framework. This model is expected to demonstrate improved explanatory power over existing linear or fragmented approaches, emphasizing the need for multidisciplinary collaboration and tailored interventions in pediatric pain care. The anticipated contribution of this research is the development of a comprehensive, evidence-based conceptual framework that can serve as a guiding tool for clinicians, educators, and policymakers. The framework aims to address current gaps in pediatric pain management by emphasizing a patient-centered, biopsychosocial approach, thereby fostering improved pain relief outcomes and quality of life for children. Furthermore, the study’s findings are expected to stimulate further research into tailored intervention strategies and evaluation metrics aligned with the proposed model. In conclusion, the study underscores the critical importance of adopting an integrated and systematic approach to pediatric pain management. Recommendations include the integration of the framework into clinical practice guidelines, the development of targeted training programs for healthcare professionals, and the formulation of policy directives that promote multidisciplinary collaboration. Future research directions should explore longitudinal validation of the framework across diverse healthcare settings and age groups, as well as the assessment of intervention efficacy based on the proposed model. Ultimately, the study aims to contribute substantively to the advancement of pediatric pain management theory and practice, ensuring that children’s pain experiences are addressed comprehensively and compassionately within healthcare systems.
