Community health outreach programs serve as vital platforms for delivering essential health services and information, yet their potential as conduits for science education remains largely underexplored. This study investigates the integration of science education within community health outreach initiatives, aiming to assess its impact on health literacy, community participation, and behavioral change. The specific objectives are to (1) examine existing models of science education embedded in health outreach activities, (2) evaluate community members' scientific literacy levels before and after exposure to integrated programs, (3) analyze the influence of science education on health-related behavioral practices, and (4) identify best practices for sustainable integration of science education into community health frameworks. The research adopts a mixed-methods case study design within a rural community of 15,000 residents served by three health outreach organizations in the region. The population comprises community members aged 18–65 who participate in outreach programs, with a targeted sample size of 200 individuals selected through stratified random sampling. Data collection instruments include structured questionnaires measuring scientific literacy and health behaviors, observation checklists, and semi-structured interview guides for health workers and community leaders. Validity and reliability of quantitative instruments are ensured through pilot testing and Cronbach’s alpha coefficients exceeding 0.8, while qualitative data are subject to thematic analysis employing NVivo software. Quantitative data are analyzed using descriptive statistics, paired t-tests to assess pre- and post-intervention changes, and multiple regression analysis to determine predictors of behavioral change. It is anticipated that the findings will demonstrate statistically significant improvements in scientific literacy scores and health behavior practices among participants exposed to integrated science education. Theoretical frameworks guiding the study include Bandura’s Social Cognitive Theory, focusing on observational learning and self-efficacy, and the Health Belief Model, emphasizing perceived benefits and barriers. This research offers significant contributions to the field by providing empirical evidence on the effectiveness of integrating science education into practical community interventions, highlighting scalable models that foster health and scientific literacy concurrently. The study also aims to fill existing gaps in literature concerning contextualized, sustainable approaches tailored to rural community settings. The main conclusion underscores that systematic integration of science education within community health outreach enhances health literacy and promotes sustainable behavioral practices. Based on these findings, recommendations are made for policymakers, health practitioners, and educators to design curricula that synergize health promotion with scientific understanding, ensuring community participation and long-term behavioral change. Future research directions include longitudinal studies to assess the sustainable impact of integrated programs and comparative analyses across diverse community contexts, thus advancing theoretical models and practical frameworks for health education and community development.
