In the context of urbanization and increasing healthcare demands, city health departments face mounting challenges in delivering efficient public health services to diverse populations. Despite significant investments in healthcare infrastructure and human resources, concerns persist regarding the effectiveness and efficiency of service delivery, often resulting in prolonged wait times, resource wastage, and unmet health needs. This study aims to evaluate the operational efficiency of the City Health Department (CHD) in providing public health services, identifying factors influencing efficiency levels, and proposing recommendations for optimization. The specific objectives include assessing the resource utilization patterns, examining the impact of organizational processes on service delivery, and quantifying the overall efficiency levels of CHD in the context of service delivery. The study adopts a mixed-methods research design, integrating quantitative data analysis with qualitative insights to provide a comprehensive evaluation. The population comprises all 1,200 staff members and 50,000 service recipients engaged with CHD over the latest fiscal year. A stratified random sampling technique is employed to select 300 staff members across various departments and 1,000 residents representing diverse demographic groups. Quantitative data collection involves structured questionnaires measuring resource inputs, service outputs, organizational factors, and client satisfaction levels, complemented by operational data obtained from the department’s administrative records. Qualitative data are gathered through semi-structured interviews with key personnel to explore organizational processes and perceptions regarding service delivery.
