The rapid digitization of information processes in small and medium-sized enterprises (SMEs) has intensified the shift from traditional paper-based document management systems to digital platforms, yet uncertainty persists regarding relative efficiency, security, and value creation across diverse SME contexts. This study addresses the problem of inconsistent evidence on performance outcomes and adoption drivers associated with digital versus paper document management in SMEs, aiming to provide a rigorous, contextually grounded assessment to inform practice and policy. The objective is to evaluate (1) operational efficiency, (2) information security and compliance, (3) user acceptance and usability, (4) cost implications, and (5) organizational learning and resilience as moderated by firm size, sector, and prior digital maturity. Grounded in the technology-organization-environment (TOE) framework and complemented by the theory of planned behavior (TPB), the research delineates how technological, organizational, and environmental factors shape system choice, implementation depth, and performance outcomes. A cross-sectional, mixed-methods design is employed across 120 SMEs drawn from manufacturing, services, and trade sectors within a metropolitan region, with 60 adopting digital document management systems (D-DMS) and 60 relying on paper-based processes (P-DMS). Quantitative data are collected through structured surveys (n = 120) measuring process efficiency indicators (cycle time, error rate, audit trail completeness), security/compliance metrics (data breach frequency, regulatory incident reports, access control strength), usability (system usability scale, task completion time), and cost components (licensing, hardware, maintenance, and personnel). Qualitative insights are gathered via semi-structured interviews (n = 24) with IT managers, operations leads, and end-users to explore perceived benefits, challenges, and organizational learning implications. Validity and reliability of instruments are ensured through pilot testing (n = 12), confirmatory factor analysis for construct validity, and Cronbach’s alpha checks (? > 0.7). Data analysis employs multivariate regression to test hypotheses on efficiency, security, and cost differentials; multigroup structural equation modeling (SEM) to examine moderation by firm size and digital maturity; and thematic analysis for interview transcripts to illuminate contextual mechanisms and user acceptance trajectories. The study contributes to knowledge by providing empirical, cross-sector evidence on the performance differentials between digital and paper document management in SMEs, clarifying the conditions under which digital systems yield superior efficiency and security without compromising usability or cost-effectiveness. It advances the TOE and TPB theoretical integration in a practical, comparable context and offers a validated measurement model for future cross-national research. Practical implications include evidence-based guidance for SME managers on selection criteria, change-management requirements, and governance structures; policy recommendations for scalable incentives to promote digital maturity; and a framework for calculating total cost of ownership and expected returns. The study concludes that digital document management can enhance SME performance when aligned with robust governance, targeted training, and scalable architectures, with the caveat that expected benefits are contingent on organizational readiness and sector-specific constraints.
