Thesis Abstract

Abstract
Fibroids are benign tumors that develop in the uterus and affect a significant number of women during their reproductive years. The management of fibroids has evolved over the years, with recent advancements in treatment options providing patients with a variety of choices based on their symptoms, reproductive plans, and overall health. This research project aims to explore both recent and orthodox treatments for fibroids, comparing their efficacy, side effects, and impact on patients' quality of life. Recent treatments for fibroids include minimally invasive procedures such as uterine artery embolization (UAE), magnetic resonance-guided focused ultrasound surgery (MRgFUS), and laparoscopic or robotic myomectomy. These techniques offer patients alternatives to traditional surgery, with shorter recovery times and less risk of complications. Additionally, hormonal therapies such as gonadotropin-releasing hormone agonists (GnRHa) are used to manage fibroid-related symptoms by inducing temporary menopause and shrinking the tumors. While these treatments are effective in reducing symptoms and fibroid size, they may have side effects such as hot flashes, bone density loss, and potential regrowth of fibroids after discontinuation. Orthodox treatments for fibroids include surgical interventions like myomectomy and hysterectomy, which are considered more definitive options for patients with severe symptoms or large fibroids. Myomectomy involves removing the fibroids while preserving the uterus, making it a suitable choice for women who wish to maintain their fertility. On the other hand, hysterectomy involves the removal of the uterus and is a permanent solution for fibroid management. While these surgeries are effective in providing long-term relief from fibroid symptoms, they carry risks such as bleeding, infection, and potential impact on fertility. The choice between recent and orthodox treatments for fibroids depends on various factors, including the patient's age, desire for future pregnancies, symptom severity, fibroid size, and overall health. Shared decision-making between patients and healthcare providers is crucial in determining the most appropriate treatment approach that aligns with the patient's goals and preferences. By understanding the benefits and limitations of both recent and orthodox treatments for fibroids, healthcare professionals can provide individualized care that optimizes patient outcomes and improves quality of life for women affected by this common gynecological condition.
