Abstract
The advancement of telehealth technologies has created new avenues for delivering healthcare services, particularly in chronic disease management for older adults. This systematic review explores the impact of telehealth interventions on the management of chronic diseases in the older adult population. The study aims to evaluate the effectiveness of telehealth in improving health outcomes, enhancing patient satisfaction, and reducing healthcare costs for older adults with chronic conditions. Chapter One provides an introduction to the research topic, discussing the background of telehealth in healthcare delivery, the problem statement surrounding chronic disease management in older adults, the objectives of the study, limitations, scope, significance, and the structure of the thesis. Definitions of key terms related to telehealth, chronic disease management, and older adults are outlined to establish a common understanding of the concepts discussed throughout the thesis. Chapter Two presents a comprehensive literature review consisting of ten key themes related to telehealth interventions in chronic disease management for older adults. The review synthesizes existing research on the benefits, challenges, and outcomes of telehealth programs targeting chronic conditions such as diabetes, hypertension, heart disease, and respiratory disorders in the older adult population. Chapter Three details the research methodology employed in this systematic review, including the research design, search strategy, inclusion and exclusion criteria, data extraction methods, quality assessment tools, and data synthesis techniques. The chapter outlines the steps taken to identify, select, and analyze relevant studies to address the research questions and objectives of the study. Chapter Four presents a detailed discussion of the findings from the systematic review, highlighting the impact of telehealth interventions on chronic disease management outcomes in older adults. The chapter examines the effectiveness of telehealth in improving disease monitoring, medication adherence, lifestyle modifications, and patient education, as well as its influence on healthcare utilization and costs in this population. In Chapter Five, the conclusion and summary of the thesis are provided, drawing together the key findings, implications, and recommendations for future research and practice. The study concludes that telehealth offers a promising approach to enhancing chronic disease management in older adults, with potential benefits for improving health outcomes, access to care, and patient engagement in self-management strategies.
