Abstract
This thesis investigates the impact of telehealth on chronic disease management in older adults within the context of home care settings. The study focuses on exploring how telehealth interventions can improve the management of chronic diseases in the elderly population, with a specific emphasis on the potential benefits, challenges, and outcomes associated with the implementation of telehealth technologies. The research methodology employed a case study approach, involving qualitative data collection methods such as interviews, observations, and document analysis. Ten older adults receiving home care services for chronic diseases were selected as participants for the study. The data collected were analyzed thematically to identify key patterns, themes, and insights related to the use of telehealth in chronic disease management. The findings of the study revealed that telehealth interventions have the potential to enhance the management of chronic diseases in older adults by improving access to care, increasing patient engagement, facilitating remote monitoring, and enhancing communication between patients and healthcare providers. However, several challenges were identified, such as technological barriers, privacy concerns, and the need for adequate training and support for both patients and healthcare professionals. The discussion of the findings delves into the implications of these results for healthcare practice, policy, and future research. Recommendations are provided for the effective implementation of telehealth interventions in home care settings to optimize chronic disease management in older adults. Overall, this study contributes to the growing body of knowledge on the integration of telehealth technologies in healthcare delivery for older adults with chronic diseases. In conclusion, this thesis highlights the importance of telehealth in improving chronic disease management in older adults within the home care setting. By addressing the identified challenges and leveraging the potential benefits of telehealth, healthcare providers can enhance the quality of care provided to this vulnerable population.
