Thesis Abstract

Abstract
This thesis investigates the impact of telehealth on chronic disease management in older adults within the context of home care settings. The study focuses on exploring how telehealth interventions can improve the management of chronic diseases in the elderly population, with a specific emphasis on the potential benefits, challenges, and outcomes associated with the implementation of telehealth technologies. The research methodology employed a case study approach, involving qualitative data collection methods such as interviews, observations, and document analysis. Ten older adults receiving home care services for chronic diseases were selected as participants for the study. The data collected were analyzed thematically to identify key patterns, themes, and insights related to the use of telehealth in chronic disease management. The findings of the study revealed that telehealth interventions have the potential to enhance the management of chronic diseases in older adults by improving access to care, increasing patient engagement, facilitating remote monitoring, and enhancing communication between patients and healthcare providers. However, several challenges were identified, such as technological barriers, privacy concerns, and the need for adequate training and support for both patients and healthcare professionals. The discussion of the findings delves into the implications of these results for healthcare practice, policy, and future research. Recommendations are provided for the effective implementation of telehealth interventions in home care settings to optimize chronic disease management in older adults. Overall, this study contributes to the growing body of knowledge on the integration of telehealth technologies in healthcare delivery for older adults with chronic diseases. In conclusion, this thesis highlights the importance of telehealth in improving chronic disease management in older adults within the home care setting. By addressing the identified challenges and leveraging the potential benefits of telehealth, healthcare providers can enhance the quality of care provided to this vulnerable population. Further research is warranted to explore additional factors influencing the adoption and effectiveness of telehealth in chronic disease management among older adults.

Thesis Overview

The research project titled "Exploring the Impact of Telehealth on Chronic Disease Management in Older Adults: A Case Study in Home Care Settings" aims to investigate the influence of telehealth interventions on the management of chronic diseases among older adults in home care settings. This study is critical due to the increasing prevalence of chronic conditions in the aging population and the potential benefits that telehealth can offer in addressing healthcare needs remotely. The project will delve into the background of telehealth and chronic disease management, highlighting the current challenges faced by older adults in managing their health conditions. By focusing on home care settings, the research aims to assess the effectiveness of telehealth technologies in providing remote monitoring, education, and support to older adults with chronic diseases. The study will identify the specific chronic diseases prevalent among older adults in home care settings and explore how telehealth interventions can improve disease management outcomes. Factors such as ease of access to care, patient engagement, and healthcare provider communication will be examined to determine the overall impact of telehealth on enhancing the quality of care for older adults with chronic conditions. Furthermore, the research will address the limitations and challenges associated with implementing telehealth solutions in home care settings, considering factors such as technological barriers, privacy concerns, and the need for caregiver support. The study will also outline the scope of the research, defining the parameters within which the investigation will be conducted. The significance of this research lies in its potential to contribute valuable insights into the role of telehealth in improving chronic disease management for older adults, ultimately enhancing their quality of life and reducing healthcare costs. By conducting a case study in home care settings, the project aims to provide practical recommendations for healthcare providers, policymakers, and technology developers to optimize telehealth interventions for this vulnerable population. In summary, this research overview sets the stage for an in-depth exploration of how telehealth can positively impact chronic disease management in older adults within the context of home care settings. Through this investigation, the project seeks to advance knowledge in the field of telehealth and geriatric care, with the ultimate goal of enhancing healthcare outcomes for older adults living with chronic conditions.
