The introduction and widespread adoption of telemedicine have revolutionized healthcare delivery, offering innovative solutions to bridge the gap between healthcare providers and patients, especially in the management of chronic diseases. This thesis explores the impact of telemedicine on chronic disease management in elderly patients through a comprehensive case study approach. The study aims to examine how telemedicine interventions can enhance the quality of care, improve patient outcomes, and address the unique challenges faced by elderly individuals with chronic conditions. The research begins with an in-depth review of the literature, highlighting the current state of telemedicine applications in chronic disease management and the specific needs of elderly patients in this context. By analyzing ten key studies, this review provides a solid foundation for understanding the potential benefits and limitations of telemedicine in addressing the healthcare needs of the elderly population with chronic diseases. The research methodology section outlines the design of the case study, including the selection criteria for participants, data collection methods, and analysis techniques. Through a mixed-methods approach, combining qualitative interviews, quantitative surveys, and telemedicine usage data, the study aims to capture a holistic view of the impact of telemedicine on elderly patients managing chronic diseases.
