Telehealth has emerged as a promising approach to improving patient outcomes, particularly in the management of chronic diseases. This thesis explores the impact of telehealth on patient outcomes in chronic disease management, with a focus on its effectiveness, benefits, challenges, and implications for healthcare delivery. The study aims to provide insights into how telehealth interventions can enhance patient care, improve health outcomes, and optimize healthcare resources. The research methodology includes a comprehensive literature review, qualitative and quantitative data analysis, and case studies to examine the role of telehealth in chronic disease management. The findings reveal that telehealth interventions can lead to improved patient outcomes, such as better disease management, increased patient engagement, and enhanced quality of life. However, challenges related to technology adoption, privacy concerns, and access to care remain significant barriers to widespread implementation.
