Telehealth has emerged as a promising tool in healthcare delivery, offering new opportunities to improve patient outcomes, particularly in chronic disease management. This systematic review aims to explore the impact of telehealth on patient outcomes in chronic disease management. The review synthesizes existing literature to evaluate the effectiveness of telehealth interventions in improving health outcomes, patient satisfaction, and healthcare delivery in the context of chronic diseases. The introduction provides the background of telehealth and its potential benefits in chronic disease management. The problem statement highlights the challenges faced in traditional healthcare delivery for chronic diseases and the need for innovative solutions like telehealth. The objectives of the study are to assess the impact of telehealth on patient outcomes, identify limitations in current research, and determine the scope of telehealth interventions in chronic disease management. The literature review encompasses ten key areas, including the definition of telehealth, chronic disease management strategies, patient engagement in telehealth, technological advancements, barriers to telehealth adoption, and the role of healthcare providers in telehealth implementation.
