This thesis investigates the potential impact of telehealth on enhancing patient outcomes in the management of chronic diseases among older adults. The study aims to explore how telehealth interventions can effectively support the management of chronic conditions in the aging population, addressing the growing need for innovative healthcare solutions. The research will focus on assessing the efficacy, feasibility, and acceptance of telehealth technologies in improving patient outcomes and quality of life for older adults with chronic diseases. The introductory chapter provides an overview of the study, presenting the background, problem statement, objectives, limitations, scope, significance, and structure of the thesis. It defines key terms related to telehealth and chronic disease management to establish a clear understanding of the research context. Chapter two conducts a comprehensive literature review, examining existing studies, theories, and evidence on telehealth interventions and their impact on chronic disease management among older adults. The review encompasses ten key themes, including the benefits, challenges, technological advancements, patient engagement, healthcare provider perspectives, and ethical considerations associated with telehealth in the context of chronic disease management.
