Telehealth applications have gained increasing attention in healthcare, offering innovative solutions to address challenges in chronic disease management, particularly in rural communities. This thesis explores the impact of telehealth on chronic disease management from a nursing perspective, focusing on rural populations. The study aims to investigate how telehealth interventions can enhance healthcare delivery, improve patient outcomes, and overcome barriers to accessing care faced by rural residents with chronic diseases. The introduction provides an overview of the growing prevalence of chronic diseases globally, emphasizing the disparities in healthcare access and outcomes experienced by rural communities. The background of the study highlights the potential of telehealth technologies to bridge these gaps, enabling remote monitoring, virtual consultations, and health education for chronic disease patients in rural areas. The problem statement identifies the challenges faced by rural residents in managing chronic conditions, such as limited access to specialized care, transportation barriers, and a lack of healthcare infrastructure.
