Telehealth, the utilization of digital information and communication technologies to provide healthcare services remotely, has emerged as a promising solution to address the challenges of chronic disease management in elderly patients within long-term care facilities. This thesis investigates the impact of telehealth on chronic disease management in the elderly population, focusing on its implementation and effectiveness within long-term care settings. The study employs a case study approach to explore the benefits, limitations, and implications of telehealth in enhancing healthcare delivery and outcomes for elderly patients with chronic diseases. The research begins with a comprehensive review of the existing literature on telehealth and chronic disease management, highlighting the key concepts, theories, and empirical evidence in this field. The literature review underscores the potential of telehealth technologies to improve access to care, enhance patient engagement, and optimize disease management strategies for elderly individuals with chronic conditions. The methodology chapter outlines the research design, data collection methods, and analysis techniques employed in this study. Through qualitative data collection methods such as interviews, observations, and document analysis, the research aims to gain insights into the experiences and perspectives of healthcare providers, patients, and caregivers involved in telehealth initiatives within long-term care facilities.
