Childhood malnutrition remains a critical public health concern in urban settings, where socioeconomic disparities and uneven access to nutritional services contribute to persistent health challenges among pediatric populations. Despite the proliferation of community health centers aiming to enhance child health outcomes through targeted nutritional programs, a comprehensive evaluation of their effectiveness, utilization, and implementation fidelity is lacking. This study aims to assess the efficacy, coverage, and factors influencing pediatric nutritional programs across selected urban community health centers, with the overarching goal of providing evidence-based recommendations for policy and practice enhancements. The specific objectives include (1) to evaluate the nutritional status of children enrolled in these programs; (2) to examine the implementation fidelity and operational capacity of the nutritional interventions; (3) to identify barriers and facilitators affecting program delivery; and (4) to analyze caregiver knowledge, attitudes, and practices concerning child nutrition. The study adopts a mixed-methods approach, integrating quantitative and qualitative data to provide a comprehensive assessment. The quantitative component employs a cross-sectional design involving 400 caregiver-child pairs selected through stratified random sampling from four urban community health centers. Data collection instruments encompass structured questionnaires on socio-demographic variables, nutritional status assessments (anthropometric measurements, screening for malnutrition), and program utilization records. Qualitative data are gathered via semi-structured interviews and focus group discussions with health workers and caregivers, exploring perceptions and operational challenges. It will fill existing knowledge gaps by systematically evaluating program effectiveness and identifying actionable factors for improvement, thereby guiding policy formulation and resource allocation. The study concludes that strengthening program implementation through targeted capacity-building, community engagement, and caregiver education is vital for enhancing pediatric nutritional outcomes. It recommends policy emphasis on integrated service delivery models, routine monitoring and evaluation, and capacity enhancement for health workers. The findings advocate for scalable interventions tailored to urban socio-cultural contexts and underscore the importance of interdisciplinary collaboration in addressing childhood malnutrition. Future research is suggested to explore longitudinal impacts and to evaluate intervention-specific strategies for sustainable nutritional improvements among children in urban environments.
