The advancement of telehealth technologies has created new avenues for delivering healthcare services, particularly in chronic disease management for older adults. This systematic review explores the impact of telehealth interventions on the management of chronic diseases in the older adult population. The study aims to evaluate the effectiveness of telehealth in improving health outcomes, enhancing patient satisfaction, and reducing healthcare costs for older adults with chronic conditions. Chapter One provides an introduction to the research topic, discussing the background of telehealth in healthcare delivery, the problem statement surrounding chronic disease management in older adults, the objectives of the study, limitations, scope, significance, and the structure of the thesis. Definitions of key terms related to telehealth, chronic disease management, and older adults are outlined to establish a common understanding of the concepts discussed throughout the thesis. Chapter Two presents a comprehensive literature review consisting of ten key themes related to telehealth interventions in chronic disease management for older adults. The review synthesizes existing research on the benefits, challenges, and outcomes of telehealth programs targeting chronic conditions such as diabetes, hypertension, heart disease, and respiratory disorders in the older adult population.
