Abstract
This thesis explores the impact of telehealth on chronic disease management in older adults. Telehealth, defined as the delivery of healthcare services remotely using electronic communication, has emerged as a promising tool to improve access to care and enhance patient outcomes. Older adults, who often face challenges in accessing traditional healthcare services due to mobility issues and chronic conditions, stand to benefit significantly from telehealth interventions. The objective of this study is to investigate how telehealth interventions can effectively support the management of chronic diseases in older adults and improve their overall health outcomes. The literature review in this thesis examines ten key studies that have explored the use of telehealth in chronic disease management among older adults. These studies provide insights into the effectiveness of telehealth interventions, the barriers and facilitators to implementation, and the impact on patient outcomes. Through a critical analysis of existing research, this thesis aims to identify gaps in the current literature and contribute new knowledge to the field. The research methodology chapter outlines the design of a mixed-methods study that will be conducted to investigate the impact of telehealth on chronic disease management in older adults. The study will involve quantitative surveys to assess patient satisfaction and health outcomes, as well as qualitative interviews with healthcare providers to explore their perspectives on telehealth implementation.
