Telehealth has emerged as a promising solution to address the challenges faced in managing chronic diseases among patients in community settings. This thesis explores the implementation of telehealth in managing chronic disease patients within community healthcare settings. The study aims to investigate the effectiveness of telehealth interventions in improving patient outcomes, enhancing access to care, and reducing healthcare costs. A comprehensive literature review was conducted to examine existing research on telehealth applications in chronic disease management. The research methodology included a mixed-methods approach, combining quantitative analysis of patient data and qualitative interviews with healthcare providers and patients. The findings revealed significant improvements in patient engagement, self-management of chronic conditions, and overall satisfaction with telehealth services. However, challenges related to technology adoption, training, and data security were identified as barriers to successful telehealth implementation. The discussion of findings highlights the implications of telehealth in transforming healthcare delivery models and the importance of addressing barriers to implementation. The study concludes with recommendations for policymakers, healthcare providers, and stakeholders to support the widespread adoption of telehealth in managing chronic diseases in community settings. This research contributes to the growing body of evidence supporting the integration of telehealth technologies in improving healthcare outcomes for chronic disease patients in community settings.
