The study aims to examine how telemedicine interventions can enhance the quality of care, improve patient outcomes, and address the unique challenges faced by elderly individuals with chronic conditions. The research begins with an in-depth review of the literature, highlighting the current state of telemedicine applications in chronic disease management and the specific needs of elderly patients in this context.
