This thesis investigates the impact of telehealth on patient care outcomes in a home care setting. Telehealth, encompassing a variety of technologies and services for delivering healthcare remotely, has gained significant attention in recent years as a means to improve access to care and enhance patient outcomes. However, its specific effects within the context of home care settings remain underexplored. This research aims to fill this gap by examining how telehealth interventions influence patient care outcomes, such as health status, satisfaction, and adherence to treatment plans, in the unique environment of home-based care. The study begins with a comprehensive literature review that explores the existing evidence on telehealth applications in various healthcare settings and identifies gaps in knowledge specific to home care. Drawing on theoretical frameworks such as the Technology Acceptance Model and the Chronic Care Model, the review highlights the potential benefits and challenges associated with telehealth implementation in home care. Methodologically, a mixed-methods approach will be employed to gather data from both patients and healthcare providers. Quantitative surveys will assess patient perceptions of telehealth, their health outcomes, and satisfaction levels, while qualitative interviews with healthcare providers will delve into their experiences with implementing telehealth interventions and the perceived impact on patient care. The findings from this research are expected to provide valuable insights into the effectiveness of telehealth in improving patient care outcomes in a home care setting. By identifying the factors that facilitate or hinder the successful implementation of telehealth programs, this study aims to inform policymakers, healthcare providers, and technology developers on best practices for integrating telehealth into home care services.
