**Abstract
** This thesis investigates the impact of telehealth on chronic disease management in older adults. As the population ages and chronic diseases become more prevalent, finding effective ways to manage these conditions is crucial. Telehealth, the use of technology to deliver healthcare remotely, has emerged as a promising solution to improve access to care and enhance patient outcomes, particularly in the older adult population. This study aims to explore the effectiveness of telehealth interventions in managing chronic diseases among older adults and to identify the factors that influence its successful implementation. The research is guided by a mixed-methods approach, combining quantitative analysis of patient health outcomes with qualitative exploration of patient and provider perspectives on telehealth interventions. A comprehensive review of the existing literature on telehealth, chronic disease management, and older adult care forms the theoretical framework for this study. The literature review highlights the potential benefits of telehealth, such as improved patient engagement, enhanced monitoring of chronic conditions, and increased access to specialized care.
