Developing a Holistic Framework for Nurse-Patient Relationship Enhancement in Chronic Care

Thesis Abstract

Effective nurse-patient relationships are pivotal to improving health outcomes in individuals with chronic illnesses, yet existing frameworks often inadequately address the multifaceted nature of these relationships within long-term care contexts. The primary aim of this study is to develop a comprehensive, holistic framework that enhances nurse-patient interactions, thereby fostering improved patient engagement, adherence, and overall well-being in chronic care settings. Specific objectives include identifying key components of effective nurse-patient relationships, exploring the influence of psychosocial, cultural, and organizational factors, and synthesizing these insights into a coherent theoretical model suitable for practical implementation. Employing a mixed-methods sequential exploratory design, the study begins with a qualitative phase involving in-depth interviews and focus group discussions with 30 nurses and 60 patients across three chronic care clinics, aimed at eliciting experiential insights into relationship dynamics. Data will be analyzed through thematic analysis using NVivo software to identify emergent themes and relationships. In the subsequent quantitative phase, a structured survey will be administered to 300 nurses and 600 patients selected via stratified random sampling, to quantify relationships among identified components and assess their predictive value on patient satisfaction and adherence. Data from the survey will be analyzed using multiple regression analysis and structural equation modeling (SEM) via Amos software to validate the conceptual relationships proposed. The study anticipates uncovering core elements such as trust, empathy, communication competence, cultural sensitivity, organizational support, and continuity of care as critical to relationship quality. It is expected that the analysis will reveal significant pathways whereby these components influence patient engagement and health outcomes, leading to the formulation of an integrative, holistic framework grounded in established theories such as King’s Theory of Goal Attainment and Pender’s Health Promotion Model. This framework aims to encapsulate individual, interpersonal, and organizational dimensions, offering a multidimensional approach to relationship enhancement. The findings are expected to contribute to the existing body of knowledge by filling a significant gap in theoretical models that comprehensively address the complexity of nurse-patient relationships in chronic care. The proposed framework provides a practical, interdisciplinary model that can be adopted by nursing professionals and healthcare administrators to improve relationship-building strategies, training curricula, and organizational policies. The study further aims to demonstrate that a holistic perspective substantially improves patient satisfaction, adherence, and ultimately health outcomes in chronic disease management. In conclusion, the study underscores the importance of adopting an integrated approach to nurse-patient relationship enhancement, emphasizing psychosocial, cultural, and systemic factors. Recommendations include the integration of the framework into nursing education and clinical practice, development of policy guidelines to support organizational structures conducive to relationship-building, and ongoing evaluation of relationship quality metrics. Future research should explore longitudinal testing of the framework in diverse healthcare settings and extend the model to incorporate technological innovations such as telehealth and digital communication tools. This study ultimately aims to serve as a foundational reference for optimizing nurse-patient interactions, thereby advancing patient-centered chronic care delivery and contributing significantly to nursing theory and practice.
