Assessing the Impact of Nurse-Led Chronic Disease Management in Rural Hospitals

Chronic diseases such as hypertension, diabetes mellitus, and chronic respiratory conditions pose significant healthcare challenges in rural hospital settings, often exacerbated by limited specialist access, resource constraints, and inadequate patient management strategies. This study investigates the impact of nurse-led chronic disease management (NCDM) programs on health outcomes, patient satisfaction, and healthcare utilization in rural hospitals, aiming to evaluate the effectiveness of nurses as autonomous providers in chronic disease care. The specific objectives are to assess the clinical outcomes of patients managed under nurse-led programs, analyze patient satisfaction levels, evaluate healthcare utilization patterns, and identify factors influencing program success or barriers to implementation. Employing a mixed-methods research design, the study integrates quantitative and qualitative approaches to provide comprehensive insights. The quantitative component adopts a quasi-experimental pretest-posttest design involving a sample of 300 adult patients with diagnosed chronic conditions receiving care at two rural hospitals over a 12-month period. Participants are selected through stratified random sampling, ensuring representation across age, gender, and disease type. Data collection encompasses clinical measurements (blood pressure, blood glucose levels, pulmonary function tests), standardized patient satisfaction surveys, and healthcare utilization records (hospital readmissions, outpatient visits). The qualitative aspect involves semi-structured interviews with 20 nurses and 20 patients to explore perceptions, experiences, and contextual factors impacting program delivery. Data validity and reliability are secured through pilot testing of instruments, triangulation, and member checking. Statistical analysis involves descriptive statistics, paired t-tests, and ANCOVA to compare pre- and post-intervention outcomes, with regression analysis employed to identify predictors of clinical improvement. Thematic analysis, guided by the Health Belief Model and Self-Efficacy Theory, interprets qualitative data to contextualize quantitative findings, elucidating barriers and facilitators to nurse-led program success. The study anticipates demonstrating significant improvements in clinical indicators, increased patient satisfaction, and reduced healthcare utilization in the intervention group compared to baseline. These findings are expected to substantiate the role of nurses in autonomous chronic disease management, aligning with global health policies promoting task-shifting and patient-centered care. The contribution to knowledge lies in providing empirical evidence on the efficacy of nurse-led interventions within resource-limited rural hospital settings, highlighting best practices and structural factors that influence success. It advances theoretical understanding by applying established health behavior theories to interpret program acceptance and adherence in rural populations. The main conclusion posits that nurse-led chronic disease management significantly enhances health outcomes, patient satisfaction, and resource optimization in rural hospitals. Recommendations include integrating nurse-led protocols into standard practice, augmenting training programs, fostering multidisciplinary collaboration, and advocating for policy adjustments to sustain nurse-led models. The study also suggests avenues for further research, such as longitudinal assessments of program sustainability and scalability across diverse rural contexts, to strengthen the evidence base for widespread implementation of nurse-led chronic disease management strategies.
