Thesis Abstract

Oral health among adolescents remains a critical public health concern due to high prevalence of dental plaque accumulation, which predisposes this age group to caries and periodontal diseases. Despite the established importance of effective oral hygiene practices, many adolescents demonstrate inadequate oral care behaviors, often due to insufficient knowledge and lack of targeted health education. This study seeks to evaluate the impact of a structured oral hygiene education program on plaque control among adolescents aged 13 to 17 years in urban school settings. The primary aim is to determine whether tailored educational interventions significantly improve plaque removal, plaque index scores, and oral hygiene behaviors. Specific objectives include assessing baseline oral hygiene practices, implementing a comprehensive educational module based on health belief models and social learning theories, and measuring changes in plaque index scores and self-reported oral hygiene behaviors over a three-month period. Employing an experimental research design with a quasi-randomized controlled trial framework, the study targeted a total population of 400 adolescents from four secondary schools, with 200 students assigned to the intervention group and 200 to the control group. Participants were selected through stratified random sampling to ensure diversity in socioeconomic status and gender. Data collection instruments included standardized plaque indices (Silness and Löe Plaque Index), structured questionnaires assessing knowledge, attitude, and practice (KAP) related to oral hygiene, and semi-structured interviews for qualitative insights. The validity and reliability of the questionnaires were established through pilot testing and Cronbach’s alpha coefficients exceeding 0.80. Data analysis employed descriptive statistics to profile baseline characteristics, paired and independent t-tests to compare within and between-group differences, and multiple regression analyses to identify predictors of plaque control. Thematic analysis was applied to qualitative interview data to explore contextual factors influencing behavioral change. It is anticipated that the educational intervention will lead to statistically significant reductions in plaque index scores in the intervention group compared to controls, alongside improved oral hygiene behaviors and increased knowledge levels. The findings are expected to support the theoretical framework modeled on the Health Belief Model and Bandura’s Social Cognitive Theory, suggesting that improved awareness and self-efficacy directly influence plaque control behaviors. The study's contributions include providing empirical evidence on the effectiveness of school-based oral hygiene education tailored for adolescents, thus filling existing gaps related to long-term behavioral change and contextual factors influencing oral hygiene practices. The main conclusion emphasizes that structured educational programs grounded in behavioral change theories can significantly improve plaque control among adolescents. Recommendations include integrating comprehensive oral health education into school curricula and training teachers as oral health ambassadors. The study further advocates for longitudinal research to assess the sustainability of behavior change and the impact on oral health outcomes over time. Overall, this research underscores the importance of combining theoretical models with tailored health education interventions to foster lasting oral hygiene practices among adolescents, thereby contributing to improved oral health status and reducing the burden of preventable dental diseases within this vulnerable population.

Thesis Overview

This research focuses on understanding how educating adolescents about proper oral hygiene can influence their ability to control dental plaque, which is a sticky film of bacteria that forms on teeth and can lead to cavities and gum disease. Despite knowing that good oral hygiene is essential for oral health, many adolescents do not maintain effective plaque control, often due to limited knowledge or motivation. The study aims to determine whether targeted oral health education can improve plaque levels among teenagers and thereby contribute to better oral health outcomes. The research addresses a gap in current knowledge about the effectiveness of specific educational interventions in adolescent populations within a real-world setting. Although some studies suggest education improves oral hygiene practices, there is limited evidence on how it translates into measurable changes in plaque control over time in this age group. To investigate this, the researcher will adopt a quasi-experimental design involving two groups of adolescents aged 12 to 16 years. A sample size of approximately 200 participants will be selected using stratified random sampling from schools. One group will receive a structured oral hygiene education program, including classroom sessions, demonstrations, and take-home materials, while the control group will receive standard oral health information. Data collection will involve clinical examination to assess plaque scores at baseline, immediately after the intervention, and three months later. Questionnaires will also be used to evaluate knowledge, attitudes, and self-reported practices. Data analysis will include descriptive statistics, paired t-tests to compare plaque scores within groups, and independent t-tests for between-group differences. Regression analysis may be used to identify factors predicting plaque control improvement. This study is expected to contribute valuable evidence on the impact of educational strategies in promoting effective plaque control among adolescents. The findings will help inform policymakers and dental health educators in designing targeted programs, with the ultimate goal of reducing oral diseases in this vulnerable age group. The main outcome anticipated is that adolescents receiving education will show significantly better plaque control than those who do not, emphasizing the importance of continued health education initiatives.
