Thesis Abstract

Early childhood caries (ECC) remains a prevalent and significant public health concern, particularly in low-income populations where access to preventive dental care is limited. Despite the extensive use of fluoride-based interventions, controversy persists regarding the optimal frequency of fluoride varnish applications necessary to effectively reduce ECC incidence among preschool children. This study aims to evaluate the impact of varying fluoride varnish application frequencies on the reduction of early childhood caries, thereby providing evidence-based guidance for dental public health practices. Specifically, the research investigates whether biannual, quarterly, or monthly fluoride varnish applications are most efficacious in decreasing caries prevalence in children aged 3 to 5 years. The research adopts a randomized controlled trial (RCT) design, enrolling a total of 300 preschool children randomly assigned to three intervention groups corresponding to three application frequencies biannual, quarterly, and monthly applications. The study population comprises children attending urban preschool facilities over a 12-month period. Data collection involves clinical oral examinations conducted at baseline, six months, and 12 months to assess caries status utilizing the decayed, missing, and filled teeth (dmft) index, complemented by parental questionnaires that gather data on oral hygiene practices, dietary habits, and socio-economic status. Furthermore, the research underscores the importance of integrating preventive strategies within community-based oral health programs, especially in resource-constrained settings. In conclusion, this study is expected to demonstrate that an increased frequency of fluoride varnish applications enhances ECC prevention, with quarterly applications emerging as a cost-effective and clinically effective regimen. It recommends that policymakers incorporate these findings into national oral health policies, advocate for regular fluoride varnish programs in preschool populations, and promote parental education to maximize prevention efforts. Future research should explore long-term outcomes beyond 12 months and evaluate the cost-effectiveness of various application schedules in diverse socio-economic contexts.
